• Title: Family Nurse Practitioner (FNP)

• Employment Type: Full-Time
• Salary: $158,000 – $185,000 annually (Based on experience) + Benefits

Position Overview

We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join a multidisciplinary healthcare team providing comprehensive medical services to patients at a community health center in San Jose.

The ideal candidate will have strong primary care experience and a passion for providing high-quality, patient-centered care, particularly within diverse and underserved communities.

-> Key Responsibilities

• Provide comprehensive primary care services, including assessment, screening, diagnosis, treatment, and preventive healthcare.
• Conduct physical examinations and obtain comprehensive patient medical histories.
• Develop and implement individualized treatment plans based on patient needs.
• Prescribe and manage medications and monitor patient outcomes.
• Educate patients on disease prevention, medication adherence, healthy lifestyles, and overall wellness.
• Collaborate with physicians, behavioral health providers, and other interdisciplinary team members to coordinate patient care.
• Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
• Coordinate referrals and consultations with specialists.
• Participate in team meetings and support team-based clinical operations.
• Provide clinician cross-coverage as needed.
• Participate in an after-hours on-call rotation.

-> Qualifications

• Master's degree or Doctorate from an accredited Nurse Practitioner program.
• Current, unrestricted California Nurse Practitioner license .
• Current board certification as a Nurse Practitioner.
• Active and unrestricted DEA license .
• Current BLS certification .
• Minimum 2 years of primary care clinical experience required.
• Experience in a community health center, FQHC, or underserved healthcare setting preferred.
• Strong clinical, communication, and patient-care skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.
